Argy Lacedom ~
AL: "Does god have equal empathy for both the boy and for the worm living in his eye, eating it from the inside out?"
No; God has much more empathy for the boy. The boy is intrinsically more valuable than the worm.
AL: "Was the boy designed as food for the worm,"
No.
AL: "or was the worm designed to blind the boy?"
No. It would not violate any divine Let's-Blind-The-Boy plan for a doctor to remove the worm and preserve the boy's sight. The affliction and suffering caused by the worm to the boy is an effect of the fall, not an effect of God's design in creation.
